$500 is provided to the four selected awardees for four years as long as criteria is met.

Provide a copy of the proof of admissions letter.

Provide a list (not a paragraph) of high school and community activities. This can be grouped by year in school or by activity. College students should provide a list of their postsecondary activities.

Provide a 200 words or less narrative highlighting education goals and reasons for seeking this scholarship.

Provide a copy of your 7-semester high school transcript. Includes 1st semester this year. (High school graduates, please include your college transcript if applicable.)

Provide a current photo.

Provide a summary of how you plan to finance your education. If you know them, use dollar amounts – tuition, room & board, etc.

Provide two letters of recommendation. These letters must be signed by the person making the recommendation.
